Hospice Marketing Liaison

Transitions Care is seeking a Hospice Marketing Liaison to build and maintain referral relationships across hospitals, clinics, senior living communities, and physician practices. In this role, you will educate providers and families about hospice and palliative care services, conduct outreach visits, present to clinical teams, and support timely, appropriate referrals. You'll collaborate closely with our interdisciplinary care team, represent our mission with professionalism and compassion, and help ensure patients and families receive dignified, holistic end-of-life support.

Responsibilities

• Develop and maintain referral relationships with hospitals, clinics, senior living communities, and physician practices

• Educate healthcare providers and families on hospice and palliative care services and eligibility

• Conduct regular outreach visits, in-services, and presentations to clinical teams and community partners

• Collaborate with the interdisciplinary care team to ensure smooth referral and admission processes

• Track, analyze, and report referral trends and market activity using CRM or similar tools

• Represent Transitions Care at community events, professional meetings, and outreach programs

• Serve as a knowledgeable resource on end-of-life care options and Transitions Care services
