Hospice Marketing Liaison

Transitions Care is seeking a Hospice Marketing Liaison to build, grow, and maintain referral relationships across hospitals, skilled nursing, assisted living, and physician practices. In this role, you will educate providers and community partners on hospice and palliative services, conduct on-site visits, and respond quickly to referral inquiries. You'll collaborate closely with our interdisciplinary clinical team to ensure smooth transitions of care, accurate communication, and strong patient and family satisfaction while meeting territory growth and outreach goals in a mission-driven environment.

Responsibilities

• Develop and maintain referral relationships with hospitals, SNFs, ALFs, and physician practices

• Educate providers, staff, and community partners on hospice and palliative services and eligibility

• Conduct regular on-site visits, presentations, and in-services to drive awareness and referrals

• Respond promptly to referral inquiries and coordinate with intake and clinical teams

• Collaborate with the interdisciplinary team to ensure smooth, timely transitions of care

• Track activity, referrals, and conversion metrics using CRM or similar tools

• Analyze territory performance and adjust outreach strategies to meet growth goals

• Represent Transitions Care at community events, health fairs, and professional meetings

• Maintain up-to-date knowledge of hospice regulations, services, and payer requirements
