Hospice Marketing Liaison

Transitions Care is seeking a Hospice Marketing Liaison to build and maintain referral relationships across hospitals, physician practices, senior living communities, and other partners. In this role, you will educate clinicians, social workers, and families about hospice and palliative services, conduct outreach visits, coordinate community presentations, and respond promptly to referral inquiries. You will collaborate closely with our interdisciplinary clinical team to ensure smooth transitions to hospice, support mission-driven growth, and represent our values of compassion, dignity, and holistic end-of-life care in the community.

Responsibilities

• Develop and maintain strong referral relationships with hospitals, physician practices, and senior living communities.

• Educate clinicians, care teams, patients, and families about hospice and palliative services and eligibility.

• Plan and conduct outreach visits, in-services, and community presentations.

• Respond promptly to referral inquiries and coordinate with intake and clinical teams for smooth transitions of care.

• Track and analyze referral trends using CRM or similar tools to inform outreach strategy.

• Collaborate with the interdisciplinary team to align growth efforts with clinical capacity and quality standards.

• Represent Transitions Care at community events, health fairs, and professional meetings.

• Ensure all marketing and outreach activities comply with hospice regulations and organizational policies.

• Provide timely reports on activities, pipeline, and results to leadership.

• Support mission-driven growth by modeling compassion, integrity, and patient- and family-centered values.
