Hospice Marketing Liaison

Transitions Care is seeking a Hospice Marketing Liaison to build and maintain referral relationships across hospitals, senior living communities, and physician practices. In this outreach-focused role, you will educate providers and families on hospice and palliative services, conduct community presentations, and support timely, appropriate referrals. You'll collaborate closely with clinical and admissions teams to ensure smooth transitions of care, track outreach and referral metrics, and represent our mission of compassionate, patient- and family-centered end-of-life care in the community.

Responsibilities

• Develop and maintain strong referral relationships with hospitals, facilities, and physician practices.

• Educate providers, patients, and families on hospice and palliative services and eligibility.

• Conduct community outreach events, presentations, and in-services to increase awareness of Transitions Care.

• Collaborate with clinical, admissions, and leadership teams to support timely, appropriate referrals and admissions.

• Track, analyze, and report outreach, referral, and conversion metrics using CRM or similar tools.

• Serve as a liaison between referral sources and internal teams to resolve issues and improve satisfaction.

• Manage a defined territory and schedule regular visits to key accounts.

• Represent Transitions Care's mission and values with professionalism and compassion in all interactions.

• Maintain up-to-date knowledge of hospice regulations, services, and industry trends.

• Participate in team meetings and contribute feedback to refine outreach and marketing strategies.
