Home Health & Hospice Sales Representative

Addus Family Company is seeking a Home Care Liaison to build and maintain referral relationships and connect patients with in-home care services. In this mission-driven role, you will partner with hospitals, clinics, and community organizations to identify eligible patients, explain Addus' personal care, home health, and hospice offerings, and coordinate smooth transitions to home. You will educate families, ensure documentation and compliance standards are met, and collaborate with clinical and care teams to support safe, high-quality, patient-focused care at home while helping grow our local service footprint.

Responsibilities

• Develop and maintain referral relationships with hospitals, clinics, and community partners

• Identify and qualify patients appropriate for home care, home health, or hospice services

• Educate patients, families, and providers on Addus services and care options

• Coordinate smooth transitions from facility to home, ensuring timely starts of care

• Complete and submit required referral, eligibility, and compliance documentation

• Collaborate with clinical and care teams to align services with patient needs

• Support growth goals by increasing referrals and admission conversion rates

• Represent Addus at community events, in-services, and outreach activities

• Maintain accurate records in electronic systems and follow all regulatory standards

• Communicate regularly with referral sources to ensure satisfaction and resolve issues Required Skills
