Home Health & Hospice Referral Sales Representative

Addus Family Company seeks a Home Care Liaison to build and maintain referral relationships, educate community partners on Addus HomeCare services, and support patient transitions to in-home care. In this mission-driven role, you will collaborate with hospitals, clinics, and social service agencies, conduct in-home assessments, and coordinate with operations and clinical teams to ensure safe, timely starts of care. You'll promote quality, compliance, and patient-centered service while helping elderly, chronically ill, and disabled individuals remain safely in their homes. Responsibilities

• Develop and maintain relationships with hospitals, clinics, and community referral sources

• Educate partners and families on Addus home care, home health, and hospice services

• Conduct or coordinate patient intakes and home assessments for service eligibility

• Facilitate smooth patient transitions from facilities to home-based care

• Collaborate with operations and clinical teams to ensure timely starts of care

• Monitor referral activity, conversion rates, and documentation accuracy

• Promote compliance with company policies and healthcare regulations

• Represent Addus at community events, meetings, and educational sessions

• Maintain accurate records in CRM or referral tracking systems

• Support quality and patient satisfaction initiatives across the service area Required Skills
